Location: WellSpan Medical Group, York, PA

Schedule: Part Time

Part Time (40 hours biweekly)

Day/Evenings

There is a part time position available that will cover mainly weekends (Saturday, Sunday and Monday) and there is also a part time position open to cover primarily evenings.

General Summary

Provides primary tele-health nursing care including triage, clinical assessment and dispensing of clinical advice based on Schmitt-Thompson protocols, critical thinking skills and clinical nursing judgement for the WellSpan Health system. This tele-nursing care is provided to all patients across the lifespan, newborns to the elderly, and directs them to the appropriate level of care, including immediate triage to EMS, the Emergency Department, Urgent Care, Primary Care provider, or home self- care guidance depending on the acuity of the reason for call and subsequent triage.

Essential Functions:

  • Provide effective tele-health nursing care and quality care management for all patients utilizing Schmitt-Thompson protocols as well as critical thinking skills and clinical judgment.

  • Utilizes in depth interview skills with age-appropriate questions to obtain critical information related to presenting symptom(s) or health concern(s).

  • Demonstrates basic usage of the nursing process through assessment, critical thinking, and clinical judgment to prioritize symptoms and/or health conditions and determines the most appropriate disposition and level of care in a timely manner.

  • Utilizes active listening and open-ended questions to obtain a comprehensive and accurate patient assessment.

  • Documents interactions in the EHR note based on evidence-based practice and standards of care.

  • Assists in the management of patient utilization of the appropriate health care resource and provides patient education on available community resources to decrease unnecessary emergency room visits.

  • Collaborates, when appropriate, with the patients' Primary Care provider regarding assessments, plan of care, disposition, and outcomes.

  • Through thorough assessment, orders appropriate medications as determined within the proper protocol and disposition.

  • Assists with other nursing functions, such as medication refills and addressing in-basket messages, that can be performed remotely during evening/night hours, weekends, and times of slower call volumes.

  • Demonstrates a commitment to patients and staff by complying with all applicable safety regulations, reporting actual and potential errors, identifying opportunities to standardize processes that will lead to increased safety; and participating in safety education programs and root cause analysis as required.

  • Maintains established policies and procedures, objectives, quality assessment, safety standards.

  • Maintains core competencies and stays abreast of healthcare changes that impact patient care, health system, and future direction.

  • Provides outstanding service to all customers, fosters teamwork by Working as One, and practices fiscal responsibility through improvement and innovation.

Minimum Education:

  • Associates Degree Required

Work Experience:

  • 3 years Broad clinical experience. Required

  • Emergency Department and triage experience. Preferred

Licenses:

  • Licensed Registered Nurse Upon Hire Required or

  • Registered Nurse Multi State License Upon Hire Required and

  • Basic Life Support Upon Hire Required

Knowledge, Skills, and Abilities:

  • Advanced triage skills.

  • Good communication, listening and assessment skills.
