We are a growing Home Healthcare organization that is dedicated to ensuring that our geriatric population has the best quality of care while living in their homes whose healthcare needs require supervision.

Position Summary:

Our organization is looking for an RN who will provide the best quality of care to our patients and the ideal candidate will have training and experience in home healthcare:


  • Completes comprehensive assessments of patient and family to determine Home Care needs at regular established timelines. Provides a complete physical assessment and history of current and previous illness (es).

  • Regularly re-evaluates patient nursing needs.

  • Initiates the plan of care and makes necessary revisions as patient status and needs change.

  • Develops the plan of care with patient’s physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Includes the patient and family in the planning process.

  • Submits all documentation on a timely basis.

  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.

  • Counsels the patient and family in meeting nursing and related needs.

  • Provides health care instructions to the patient/family as appropriate per assessment and plan.

  • Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient.

  • Participates in on-call duties as defined by the on-call policy.

  • Ensures that arrangements for equipment and other necessary supplies and services are available.

  • Instructs, supervises and evaluates home health aide care provided as stipulated by law and regulation.

  • Prepares clinical notes and updates the primary physician when necessary.

  • Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required.

  • Communicates with Case Managers and other health related persons after new patient admission, resumption of care, and whenever the patient status changes.

  • Anticipates the needs of the patient, and recommends changes to Doctor and family as appropriate

  • Notifies Agency of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, as per agency policies.

  • Investigates and reports all incidents and complaints as per agency policies

  • Interacts with DON and Administrator as appropriate

  • Interacts with family members and representatives in a courteous and professional fashion at all times

  • Assures that all logs are maintained as required by Agency

  • Provides coverage for any other agency site as so requested by management

  • Complies with all other requests from management


  • Home Healthcare experience strongly preferred

  • Valid and current Florida RN License required

  • Associate’s Degree in nursing required - BSN and MSN preferred

  • At least minimum of 2 years of recent homecare or med surge hospital experience preferred

  • CEU’s to include – HIV, Domestic Violence and Alzheimer’s - required

  • Level II Fingerprint Screening are completed and active - required

  • CPR/BLS - required

  • Updated TB/PPD - required
