Job Description:

The On-Call Registered Nurse – Hospice will coordinate the after hours healthcare needs of hospice patients. This position is responsible for promptly answering and triaging calls from patients or family members, physicians and others. This individual will follow established procedures in performing visits as necessary and will initiate communication with the case manager, attending physician or other staff as needed to coordinate optimal care and use of resources for the patient/family, as well as provide appropriate support at time of death and perform bereavement assessment if requested.

Essential Duties & Responsibilities including but not limited to:

· Respond to telephone requests for assistance and perform home visits for hospice and palliative care patients and families in their home setting after-hours and on weekends.

· Answer and respond to incoming calls, referrals, and requests for assistance.

· Communicate significant findings, problems, patient's needs, reports changes in condition or environment to the physician, clinical manager and other members of the hospice care team and obtains physician orders as required.

· Maintain flow of communication between in-house staff, field staff and other departments, for coordination of activities and services.

· Participate as a member of the interdisciplinary team.

· Accepts accountability for patient/ family satisfaction scores and quality of care outcome measure results.

· Respects and supports patient and family’s cultural beliefs and values, individual needs, goals and treatment preferences.

· Provides health care instructions to the patient / caregiver as appropriate per assessment and plan of care.

· Document accurately and appropriately all communication from physicians, clinicians, caregivers and patients, etc., as a Case Communication in the patient’s chart in real time.

· Clinical visit notes describe the assessment findings, interventions, and response

· Completes documentation in accordance with agency timeliness policies.

· Maintains and updates clinical knowledge and skills based on current nursing practice.

· Adheres to departmental requirements for required training.

· Participates in the weekend/holiday/on-call rotation as defined by Beth Israel Lahey Health at Home.
