JOB REQUIREMENTS: PLEASE NOTE: This position offers the opportunity to earn supervised clinical hours. This CCS Service Facilitator position will be assigned to a special team that focuses on flexible, intensive services for youth. Members of this team work a flexed schedule that may include at least one evening per week depending on the needs of the team. Team members also participate in and receive compensation for an on-call rotation to cover after hours emergencies for identified clients. These positions have a reduced caseload and receive additional training and supervision. Services will be tailored to the unique needs of the individual in such a way as to support and encourage independent functioning and personal choice while ensuring that basic living needs are being adequately addressed. Service Facilitator will provide counseling, supportive follow along services, consultation and assist individuals in accessing needed programs to assist them in the process of recovery and stabilization in their community. RESPONSIBILITIES A. ESSENTIAL FUNCTIONS 1. Provide recovery-based individualized case management services to children and adults with mental health and AODA issues. Adhere to definition of case management as defined by Wisconsin Administrative Code DHS 36 and Medical Assistance Guidelines. Work collaboratively with consumers and their recovery teams in assessment and in developing integrated service plans. Provide linkage and collaborate with contracted service providers, problem solve, advocate, monitor and follow up on services delivered to the consumer. Use strategies including strengths assessment, establishing hope and facilitating empowerment for consumers in individual and group approaches. 2. Within parameters established within DHS 36 and Medical Assistance guidelines, complete required documentation, to include but not limited to assessments, treatment plans, contact notes, functional screen eligibility, and training and supervision logs. Review vendor monthly invoices and documentation to assure accuracy of billing, and compliance with recording requirements. 3. Participate as a member of a multi-disciplinary team and attend recovery team meetings regarding the CCS/CRS consumers. Please visit https://careers.woodcountywi.gov/ for complete list of duties. ***** OTHER EXPERIENCE AND QUALIFICATIONS: -Two years of exper ence providing mental health services to the seriously mentally ill is preferred. -Knowledge and experience working with co-occurring (mental illness and alcohol and other substance abuse) disorders is preferred. -Knowledge of recovery-based principles and services is desirable. -Position requires the ability to work effectively with a variety of other persons and requires the ability to work and manage own time effectively. -Good oral and written communication skills are vital. -Candidates must possess basic everyday living skills and the ability to understand and follow directions, the ability to read and write (for reports), and the ability to interview and counsel. -This position may require travel to and from off-campus sites.
