Position Overview: The Master Social Worker (MSW) serves both the in-center hemodialysis department and the outpatient nephrology department. In this role, the MSW provides case management and care coordination for patients with end stage renal disease who are receiving hemodialysis as well as patients seeking nephrology care for any stage of kidney disease or kidney function problem. The MSW coordinates with the other members of the interdisciplinary hemodialysis/nephrology team; which includes the Medical Director, Nephrologist, Nurse Practitioner, nursing staff, dialysis technicians, dietitian, administrative support, and management. The MSW also interacts with a variety of community organizations and services to support the needs of the patients served. The MSW will periodically attend the Dialysis Quality Assessment and Performance Improvement team. The MSW also participates in departmental and system-wide quality improvement activities.
