Job Title: Intake Coordinator - Residential/SILP Department: Residential Treatment

Position Summary: Assume direct responsibility for and administer an intake and referral process which ensures the identification of client needs and the determination of a reasonable expectation that Gateway-Longview's programs can be adapted to meet those needs

Gateway Longview’s Residential Treatment Services is a therapeutic intervention for youth ages 12-18 and supports them in developing the skills needed to successfully live with their families or in the community.

Bachelor's Degree: $21.30 - $23.38 per hour

Position Specific Duties/Responsibilities:

  • Ensure full utilization of Supervised Independent Living Services and Residential Services by coordinating the processing of new intakes with Counties requesting placements. Assume direct responsibility for referral and intake. This includes analysis, evaluation, and implementation of necessary changes. Maintain ongoing contact with referral sources to ensure effective communication and constant flow of referrals. Complete intake interview and assessments.

  • Work collaboratively with program directors and other staff to maintain an ongoing awareness of current and emerging placement resources and client needs. Utilize this knowledge to assure responsiveness to these resources and needs. Participate in Special Treatment Team meetings as assigned.

  • Facilitate scheduling and participation in case conferences/CFT’s.

  • Assist in the development, implementation and monitoring of an accurate and effective documentation system.

  • Complete required reports and documentation in timely manner. (Includes, but is not limited to: strategic mapping and outcome results and case record review). Maintain any needed data associated with completion of these reports.

  • Oversee strategies for sustainability of Solution Focused Trauma Informed Care concepts and techniques. Function as liaison with other internal sustainability leaders within the agency.

  • In collaboration with the Clinical Director and the other Residential Administrators, effectively lead the program as required to support and advance its mission.

  • Other duties as assigned.

  • Demonstrated competence and knowledge of clinical issues of emotionally disturbed children and their families

  • Ability to work effectively with clients, staff, families, and community contacts

Required Qualifications

  • Bachelor’s degree in Social Work or related field with 5 years of experience

  • Valid NYS Driver’s License

Preferred Qualifications

  • Master’s degree in Social Worker or related field preferred
