Harry Hynes Memorial Hospice was founded by community volunteers in 1983 as a nonprofit organization, we are honored to be a leading provider of Hospice and Palliative care in South Central and Southeast Kansas. Having served more than 40,000 patients and family members, we are experts in specialized care designed to provide support to you and your loved ones during an advanced illness. Our team of professionals, including a medical director, nurses, hospice aides, social workers, and volunteers, work in coordination with the patients physician to provide pain and symptom management, as well as social and spiritual support.

Summary

The Certified Home Health Aide (HHA) is responsible for assisting patients and their family members with personal care tasks as assigned and supervised by the registered nurse.

Responsibilities

  • Assists in maintaining the patients physical comfort and hygiene, as directed by the RN.
  • Provides appropriate type of bath according to the patients condition and HHA care plan.
  • Provides oral hygiene, as directed.
  • Provides personal grooming, as directed.
  • Promotes personal grooming, as directed.
  • Promotes optimal skin integrity by providing skin care, as directed.
  • Performs non-skilled nursing procedures, as ordered by the physician and directed by the RN.
  • Assists patient with ambulation, transfers, and range of motion, as directed.
  • Performs all tasks assigned per the patients plan of care; report to RN any tasks not completed at time of service.
  • Maintains a clean, safe and comfortable environment for the patient.
  • Performs housekeeping duties, when directed by the RN.
  • Changes patients bed linens, as directed by the RN.
  • Demonstrates appropriate us of standard precautions.
  • Contributes to continuity of care and effective communication between hospice team members.
  • Accurately assesses and records patients vital signs when directed by RN.
  • Reports significant physical and/or emotional changes in patient/family condition to the appropriate interdisciplinary team member and/or RN.
  • Accurately document care provided in a timely manner per company policy.
  • Communicates in a clear and courteous manner with patients/families and staff involved with patients care.
  • Complies with the Federal HHA Standards by attending a minimum of 12 hours of in-service per calendar year.

Qualifications, Education and Experience

  • Minimum one (1) year previous home health aide experience, preferred.
  • Successful completion of an approved HHA program.
  • Must have demonstrated ability to communicate effectively.
  • Current State of Kansas HHA certificate.
  • Valid Kansas Drivers license and proof of current auto insurance.
  • High School Diploma or GED.
