Schedule Weekly Hours:40 Job Description: Gundersen Tri-County Hospital and Clinics is seeking a full time Nurse Practitioner or Physician Assistant. This position will work 0.5 FTE in the clinic and 0.5 FTE in transitional care/swing beds. Join a team of family medicine physicians and nurse practitioners to provide high-quality, person-centered primary care in Trempealeau County. The primary clinic location will be Whitehall. Practice highlights Monday-Friday, clinic hours schedule. Will likely spend some time on Transitional Care unit and some time in clinic each day-position requires flexibility to meet the demands of transitional care unit which may vary from day to day and week to week. Responsible for up to 12 patients in our Transitional Care Unit overseeing chronic medical conditions and recovery from any acute conditions while they work with our team of nurses and rehabilitation staff to eventually return home. Time spent in clinic will focus on same day/acute care, hospital or ER follow-up, preoperative history and physicals and other needs that may arise.
