Cleveland Clinic RN Case Manager - Per Diem in Weston, Florida

As a Registered Nurse (RN) Case Manager, you will work collaboratively with inter-professional teams across the continuum of care to facilitate and ensure effective transitional care coordination. Essential to this role are the core values of patient advocacy, holistic assessment, care planning and the empowerment of patients and families to self-manage. Case Managers also ensure compliance with the standards of the accrediting bodies, state mandated revised code and the CMS Conditions of Participation. The ideal future caregiver is someone who: * Can work 2 days per week and 1 weekend day a month * Is a Registered Nurse (RN) with recent inpatient, hospital bedside, and/or emergency department experience. * Is a decision-maker who can work both independently and as part of a larger team in a fast-paced environment. * Has excellent communication, multitasking and organization skills. * Is compassionate and empathetic. * Is self-motivated and detail oriented. Job Responsibilities:
Promotes inter-professional synchronization throughout the acute episode, and completes effective handoff to the next level of care or community to ensure continuity of care
Proactively assesses, identifies, and works to mitigate the risk posed by barriers to the patients' transitions of care.
Works in partnership with the patient and/or care partner(s) to develop a comprehensive, individualized transitional care plan. Participation and leadership in patient rounding activities, huddles, and committee meetings is expected, where appropriate. Education:
Graduate from an accredited school of professional nursing.
BSN preferred. Certifications:
Current State Licensed Registered Nurse (RN).
Certification by an approved entity preferred within one year of eligibility, e.g. CCTM, ACM, CCM. American Heart Association.
Basic Life Support (BLS) through American Heart Association (AHA) Competencies (Complexity of Work):
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action. Work Experience:
Minimum of three years of recent RN clinical experience required. Minimum of two years of recent experience in Utilization Review, Quality, or Care Management preferred.
Knowledge of acute care regulatory/accreditation requirements preferred. Discharge planning and/or Quality Improvement experience preferred Physical Requirements:
Requires normal or corrected hearing and vision to normal range.
Ability to perform work in a stationary position for extended periods.
Ability to communicate and exchange accurate information
Ability to work with physical records, such as retrieving and filing. Ability to operate a computer and other office equipment.
Ability to travel throughout the hospital system.
