Position Purpose As a Social Worker with the PACE Organization (PO), you will deliver comprehensive social services to participants and their families. Your role will include assessments, treatment, counseling, education, resource provision, and ongoing case management.

What You Will Do

  • Deliver Care: Assess and provide treatment, counseling, and education to participants and their families.

  • Advocate & Support: Ensure participant and caregiver needs are met through advocacy and support, including mental health needs and resource coordination.

  • Team Collaboration: Work as part of an interdisciplinary team to develop and implement care plans.

  • Case Management: Conduct assessments, maintain detailed case records, and handle home visits and financial eligibility assessments.

  • Crisis Intervention: Provide crisis support and assist with end-of-life planning, hospice care, and legal directives.

Minimal Qualifications

  • Master’s degree in Social Work with at least 2 years of experience, including working with the elderly or frail population.

  • Current social work license preferred.

  • Experience with multidisciplinary teams and knowledge of social service principles.

  • Familiarity with Medicare, Medicaid, and related financing mechanisms.

  • Valid driver's license and reliable transportation.

  • Strong interpersonal and communication skills, both written and verbal.
