What You'll Do
Collaborate with a team of dedicated social workers to plan, organize, and implement the Social Service Department's operations, ensuring the individual needs of residents are met according to established standards and regulations.
Conduct private interviews with residents and families to understand their social history, interests, and needs, and actively participate in community planning to enhance their experience.
Mentor and coach personnel from other departments, fostering excellence and addressing concerns with courageous conversations to maintain a harmonious environment.
Identify and communicate healthcare instructions, ensuring that residents' preferences and advance directives are considered.
Assist in developing and implementing social care plans and resident assessments, taking into account cognitive evaluations and resident moods to provide comprehensive follow-up care.
Maintain up-to-date administrative records and submit necessary reports promptly.
What You'll Bring
A bachelor's or master's degree in social work or a related human services field,
Previous social work experience in a long-term care setting.
State social worker licensure and prior experience in long-term care or with geriatric populations is preferred.
Excellent interpersonal skills for effective collaboration with cross-functional teams, customers, and end-users.
Strong self-motivation, energy, and the ability to work independently.
Superior written and verbal communication skills and proficiency in Microsoft Office suite.
High personal accountability and decision-making abilities to interpret and implement organizational goals, policies, and procedures.
