FirstHealth of the Carolinas Patient Care Coordinator in West End, North Carolina

Reviews and approves all plans of care. Approves all revisions to the plan of care. Approves all recertifications and discharges. Ensures all appropriate disciplines are utilized. Monitors utilization of services including supplies.Makes recommendations for efficient and effective utilization of resources to achieve positive patient outcomes. Ensures patient's goals are appropriate, meaningful and reachable. Ensures positive clinical as well as financial outcomes.Participates in the performance improvement process as directed.

RN with a current NC license or RN multi-state licensure privilege through the Nurse Licensure Compact. Bachelor's Degree in Nursing preferred. Two-three years of home health experience is required. Case Management experience required. Excellent working knowledge of OASIS and related outcomes and PPS required. Certification in OASIS required within 3 years of hire. Must have excellent clinical and critical thinking skills. Must have excellent customer service skills and work well with patients, physicians and other referral sources. Must have excellent knowledge of home health coverage criteria and service area. Must be highly organized and detail oriented.
