JOB REQUIREMENTS: Will provide support, guidance and assistance to 3 residents with mental health issues &/or developmental disabilities in a duplex setting. Our goal is to demonstrate, promote & teach skills necessary for independent functioning, healthy living and a positive self-esteem, while maintaining a safe and secure environment. Responsibilities: * Promote and facilitate resident interaction & positive attitudes * Supervise and assist with all activities of daily living (personal hygiene, home cares, meal planning & prep) * Monitor & assist with medications, treatments & general health care * Provide support and symptom monitoring * Transport residents to appointments & shopping in community * Arrange and facilitate structured community activities (ex. dinner outings, movies, bowling, picnics) * Complete daily documentation on residents & provide shift to shift communication between staff * Attend monthly staff meetings and complete continuing education requirements ***** OTHER EXPERIENCE AND QUALIFICATIONS: WILL TRAIN!!! * Must be: patient, caring, honest, flexible, self-motivated, dependable & enthusiastic. * Must be: a good listener, good leader, an enabler, a collaborator, and approachable with good communication skills. * Must be able to demonstrate good time management & problem solving skills. * Must have a safe & responsible driving history that will be acceptable by the company\'s insurance carrier. * Must pass criminal, caregiver and driver background checks * Must have good reading, comprehension & writing skills (typing & computer skills are helpful, but not required) * CBRF, Supportive Home Care or nursing assistant certification helpful, but not required. ***** APPLICATION INSTRUCTIONS: Call For Appointment: (262)335-3789 E-Mail a Rsum: biskonancy@att.net
