B/S/S/T Area Agency on Aging, Inc. has a vacancy for an Aging Care Manager 1 in our Wellsboro, PA office. This is a full time position (40 hrs. a week; Monday-Friday 8am-4:30pm) with benefits including health insurance, vision and dental insurance along with voluntary supplemental insurance.  This full time position comes with paid time off (vacation, sick, holiday and personal) and 401(k) with an Agency contribution after a year of employment.

The B/S/S/T Area Agency on Aging is looking for someone who has a passion to work with older adults, veterans and the disabled population.  This individual must be a strong consumer advocate who can maintain a balance of objectivity and empathy.  The following skills are needed: have excellent communication skills (both written and verbal); have advanced computer skills; have excellent documentation skills; have the ability to plan and organize work effectively; have strong problem solving skills; and promote a positive and professional image to the public and professional community.   

Responsibilities include: maintain a care management caseload; take referrals; conduct home visits; complete assessments; develop care plans; arrange for services; monitor ongoing needs of individuals; serve as a link to community services; maintain cooperative working relationships with service providers; and ensure services are being provided as directed.   

Travel is required throughout a specific demographic area with mileage reimbursement. Must have a reliable vehicle for work related travel, when an Agency vehicle is not available.  Must have up-to-date car registration, up-to-date car insurance and possess a valid driver's license.

Selected applicant must pass a state police criminal clearance, child abuse clearance and if necessary, a FBI clearance. These clearances will be obtained with assistance from the Area Agency on Aging, and free of charge to the selected individual.

Minimum Experience and Training Requirements:

Bachelor's Degree which includes 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences;

Or

Three and one-half years of experience as an Aging Case Aide and 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences;

Or

Any equivalent combination of experience and training which includes 12 college-level credit hours in sociology, social welfare, psychology, gerontology, or other related social sciences.

*Once an individual is hired and has successfully completed six months of being an Aging Care Manage 1, the employee will then be reclassified to an Aging Care Manager 2. *
