Job Summary:

The Supports Coordinator Assistant - Family Services serves as an advocate and broker of supports and services for children and adults with intellectual and/or developmental disabilities. Supports Coordination Assistant -- Family Services shall promote community inclusion, full citizenship, the generation of income, and networks of natural support. The Supports Coordinator Assistant - Family Services will develop resources and supports that result in the person living the life they wish to lead, being happy, healthy, and having solid relationships. The Supports Coordinator Assistant - Family Services. The Supports Coordinator Assistant focuses efforts on connecting the people supported to networks of family, friends, and meaningful relationships, and develops strategies to coordinate the individual's personal resources with entitlements and special programs to provide economic and social place in the community.  The Supports Coordinator Assistant provides these services while adhering to Person Centered Planning, Family Centered Planning and the principles of Self-Determination, the Michigan Mental Health Code, Detroit Wayne Integrated Health Network (DWIHN) and Michigan Department of Health and Human Services (MDHHS) requirements, Self-Directed Services Technical Advisory,  CLS Guiding Principles, Code of Ethics, and professional code of ethics.
