Communitas, Inc. Residential Registered Nurse RN in Wakefield, Massachusetts

We are looking for an RN Health Care Coordinator to provide support services to our individuals who reside in our community group homes.

What you will do

The Health Care Coordinator will be responsible for the oversight of the overall healthcare and well-being of individuals who reside in our Residential Program group homes and independent apartment settings. We are looking for someone with a strong background and experience in working with adults with Developmental and Intellectual Disabilities. Responsibilities include staff training on specific treatment protocols and providing consultation regarding the medical needs of the individuals. Effective communication skills and sound judgement skills are a must along with the ability to be a team player with a strong work ethic. Certified MAP Trainer is a plus but training is also available

  • The annual salary for this position is $91,520 per year.

  • This is a full time position working 40 hours per week.

  • Typically this position will operate on a Monday - Friday schedule. For emergency healthcare needs, the HCC might be required to work in the evening or on a weekend.

  • Schedule may vary, must have flexibility with the scheduling as the days and times need to meet the requirements of the agency and the individuals' needs.

  • Providing support to staff via text or phone is required.

Qualifications and Skills

  • Must have a current Massachusetts license as a Registered Nurse (RN).

  • Must have at least 3 to 5 years’ experience in a community health setting; preferably with adults with developmental and intellectual disabilities.

  • Must respect the dignity of individuals with disabilities and believe in their self-determination and growth.

  • A valid driver's license, acceptable driving record and reliable transportation is required.

  • Successful candidates will be required to complete a background check including references, CORI, and fingerprinting.
