Responsibilities and Duties ·       Provide care based on an individualized plan of care prepared by a registered nurse. ·        Provide companionship and personalized care while encouraging independence and self-care. ·        Assist clients with activities of daily living, including dressing, bathing, grooming, toileting, and eating. ·        Provide mobility and transfer assistance ·        Prepare meals and snacks. May include grocery shopping. ·        Perform light housekeeping, including vacuuming, laundry, and linen changes. ·        Assist with incontinence care. ·        Assist with transportation needs.
