Tri-County Health Care Diabetic Educator - Registered Dietitian in Wadena, Minnesota

Astera Health is looking for a Diabetic Educator that is passionate about sharing their knowledge to the inpatient and outpatient setting!

This position is 24-36 hours per week between the times of 7:30am to 5:00pm. Occasional weekend/evening coverage for community events and Diabetic Education class offered to community as needed. Responsibilities include, but are not limited to:

  • Provide education related to diabetes and the disease process; medication, diet, foot care, prevention and care of complications, blood glucose monitoring and techniques of self monitoring and injections.

  • Performs data collection on outcome indicators for performance improvement to evaluate the effectiveness of the Diabetes self-management education (DMSE) program.

  • Assist and plan Community benefit events as assigned.

  • Assists with annual program review based on the American Diabetes Association (ADA) requirements.

  • Serves as a resource for hospital and medical staff regarding the care and treatment of patients with diabetes.

  • Collaborates with team members and medical staff to meet the needs of the patients and community.

  • Coordinate annual staff competencies regarding glucose monitoring; serves as a trainer for this competency.

    Competencies required:

  • Distinguishes priorities in patient care

  • Demonstrate problem solving skills and autonomy

  • Strong oral and written communication skills.

  • Demonstrates high degree of commitment to customer service excellence.

    Education and experience required:

  • Bachelors degree in Dietetics or Nutrition with active Registered Dietitian license

  • Certified Diabetes Educator or ability to obtain certification preferred.
