LPN Primary Care Clinic Location: Juneau, AK Duration: Fulltime Job Description: Coordinates a variety of nursing care procedures for patients of all ages requiring professional knowledge and consideration of specific patient conditions and treatment. Participates in patient and family teaching. Provides leadership by working cooperatively with ancillary nursing and other members of the healthcare team in maintaining standards for professional nursing practice in the clinical setting under the supervision of a licensed registered nurse (RN). Guidelines include physician’s orders, standards of care, nursing policies and procedures, manuals, hospital/long-term care policies, and supervising RN judgment. Is legally responsible for individual practice and may incur liability for deviation from safe practice. Maintains responsibility for all actions taken. Consults with appropriate manager/supervisor in difficult situations requiring additional input. Makes informed decisions based on factual and objective data that has been analyzed for all pertinent information. Reacts in a calm manner and makes sound nursing judgments in an emergency environment. Is accountable for accepting delegation from supervising RN and for own actions in carrying out nursing care and procedures. Consults with appropriate manager/supervisor in difficult situations requiring additional input. Key Essential Functions and Accountabilities of General LPN Promotes the SEARHC Seven Standards of Excellence Implements safe, therapeutic, and efficient care for patients with needs, due to multisystem disease and/or complications of treatment, under the supervision of an RN, using appropriate infection control measures. Sets priorities for care of patients based on acuity and/or patients’ preference. Demonstrates knowledge of equipment use. Accurately administers age-specific medications (oral, topical, and intramuscular) and therapeutics (examples include, but are not limited to, oxygen therapy, sterile and un-sterile dressings, catheterizations, IV starts, irrigations, etc.). Carries out medical and nursing care plan and coordinates patient care activities with RNs, physicians, and other healthcare team members. Facilitates healing by providing nursing care that is individualized, goal oriented, and based on the nursing process. Seeks out resources when beyond the scope of skill, knowledge, or experience. Follows through on approved nursing care plans by teaching and demonstrating skills essential for understanding and coping with illness and promoting optimal health. Documents patient’s response to care and assures confidentiality of patient information. Obtains, interprets, and applies age-related (pediatric, adult, and geriatric) patient information in terms of cognitive, physical, emotional, and normal growth/development needs of the patient. Documents assessment findings that are consistent with other providers. Distinguishes between normal and abnormal assessments to identify patient capabilities and limitations including psychosocial risks and reports findings/changes to supervising RN. Imparts educational knowledge to develop skills to enhance decision making and promote understanding by interacting with patients, family, staff, and community. Engages in ongoing evaluation of effectiveness of teaching in terms of patient understanding and behavioral outcomes. Organizes and participates in patient care conferences as applicable. Develops/implements educational programs and learning aids in the department (i.e., in-services, special projects, learning aids, patient care standards). Documents education accurately and completely Provides discharge patient information. Participates in in-services and educational programs in the department, hospital, and/or community as assigned. Completes all mandatory training and competency checks. Communicates with staff, patients, significant others, and other members of the healthcare team in a way that is supportive, constructive, and positive. Gives feedback to other staff and healthcare team members regarding behavior that does not support achievement. Assists in the orientation of new staff, travelers, and students by sharing knowledge, experience, and verbal encouragement. Supports the area and overall nursing services mission by assigning staff to productive tasks and floating to other units as needed. Provides input regarding the need for change in policies, procedures, and patient care standards; accomplishes all assigned patient care and area support activities during the shift. Transcribes and notes physician orders accurately. Utilizes experience to provide a range of options for nursing care Writes progress notes in electronic health record (EHR). Gives shift report according to area guidelines. Maintains a clean and safe working environment. Maintains/stocks adequate supplies. Maintains equipment. Reports/removes defective or missing equipment and safety hazards. Adheres to infection control policy and procedures. Maintains annual competencies including safety and infection control requirements. Attends and participates in at least 50% area staff meetings annually. Applies information presented in staff meetings. Participates in unit goal setting. Complies with area and departmental scheduling guidelines. Initiates discussions with nurse manager regarding area or department issues. Complies with dress code. Refers un-resolved problems with patients/visitors/co-workers to Nurse Manager or Administrative Manager for resolution.
