About the Position

The position is primarily responsible for providing end-of-life care and hospice services to patients in their home or community setting after regular business hours, weekends, and/or holidays.

Job Duties

    1. Assesses need and provides support by phone or a visit, depending on acuity of needs and in accordance with standards of practice and evidence-based knowledge for hospice programs set by the National Hospice and Palliative Care Organization.

    2. Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family.

    3. Obtain data on physical, psychological, social and spiritual factors that may influence patient health status and incorporate that into the plan of care.

    4. Assist in determining the need to change the level of care being provided for hospice patients based on assessment and collaboration with attending physician and/or hospice medical director and manager on-call.

    5. Provide or arrange for delivery of medications and supplies needed after business hours, weekends or holidays.

    6. Provide appropriate support at the time of death, per agency policy, and support the patient’s and family’s unique spiritual and cultural beliefs.

    7. By end of shift, report on-call activities to the hospice team via email, including phone calls, communication, visits, change in condition and other pertinent clinical information.

    8. Documents in the electronic record all clinical nursing visits, medication additions and changes, care plans, and additional documentation per protocol within 24 hours of provided service.

    9. Teaches or supervises the teaching of household members, facility staff and volunteers who are responsible for assisting the resident with nursing or personal care needs.

    10. Competent in evidence-based goals of care conversations with residents and families.

    11. Triages referral calls and assesses whether patient needs immediate admission. Is able to competently complete all parts of the admission process, including receiving physician certification, if needed.

    12. Per shift, informs the Director of Clinical Services or Administrator of unusual or potentially problematic resident/family issues.

    13. Responds calmly and tactfully in stressful and emergency medical and psychosocial situations.

    14. Understands the necessity of full partnership with staff at all skilled nursing facilities, including actively soliciting feedback, responding appropriately and educating tactfully.

Job Requirements

    1. A registered nurse who is currently licensed to work in the state of Virginia.

    2. Minimum 5 years experience as a professional nurse, preferably in a hospice or in an acute care or long-term care setting.

    3. Certified Hospice and Palliative Care, preferred.

    4. Demonstrated ability to assess and respond to the needs of terminally ill residents and families in varied settings.

    5. Display a positive and professional image and attitude in all relationships with patients, families, peers and in the community.

    6. Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

    7. Proficient in Word, and able to master documentation software competently enough to assist others in its use.

    8. Must be able to work independently, have demonstrated patient care planning and patient teaching skills and strong written and verbal communication skills.

    9. Current CPR certification.

    10. Valid drivers license and daily access to a reliable and insured vehicle.

    11. Belief in the hospice concept.

    12. Participate in the Quality Assessment Performance Improvement Program (QAPI), as requested.

    13. Participate in orientation of new employees or volunteers, as requested.
