We help provide relief from the burdensome symptoms of chronic illness, help patients and caregivers navigate the healthcare system, and guide patients through difficult treatment choices. About our Advanced Practice Provider Role

Upperline Health is seeking Advanced Practice Providers for remote-based care in one (or preferably multiple) of our states including AL, CA, FL, GA, IN, KY and TN.

In this role, the APP will conduct telehealth assessments serving a primarily geriatric patient population through evidence-based care with the aim of driving improved patient outcomes. The provider will work with physicians, specialists and other members of a multidisciplinary team to identify high risk patients and develop a care delivery pathway that addresses patients’ physical, psychological and environmental needs.

APPs develop plans of care in accordance with the patient’s health status and overall goals and values; provide medical diagnostics and treatments in the home consistent with the APP’s scope of practice and the patient’s medical condition; educate patients and families on medication usage, diet and nutrition, and crisis anticipation and prevention; communicate medical information to the patient’s physicians and other providers as well as to Upperline’s Lead Physician and broader clinical team. 

Ideal candidates will have experience working with Medicare patients. Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment.


Responsibilities of the Advanced Practice Provider

  • Completes full assessment via telehealth of patient including diagnosis and evaluation of all medical conditions, assessment of the home environment, medication review, and identification of needed referrals. Addresses any issues identified during the course of the visit.

  • Differentiates normal aging from illness and disease processes and uses standardized assessment processes to assess social support and health status such as cognition, mobility, pain, skin integrity, quality of life, nutrition, neglect, and abuse.

  • Identify need and frequency of visits based on patient’s health status and articulate need to social work /nurse Health Coaches as deemed appropriate.

  • Work with patients and families to improve their quality of life, reduce health risk factors and mitigate negative outcomes.

  • Implement clinical pathways and adhere to best practices that yield compliance and improves outcome for each patient.

  • Educate patients and families and support them in assuming responsibility for their health by reducing further exacerbation of their disease, improve their overall health status and attain their goals.

  • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention.

  • Communicate, consult, and collaborate with physician partners in the delivery of quality patient care protocols and practices; with the aim of reducing hospital readmissions, ED utilization, and closing care gaps.

  • Be a strong patient advocate and work to minimize fragmented care within the care delivery system, to ensure all services provided to patients are seamless and efficient.

  • Maintain patient medical records and medical documentation consistent with state regulations and company standards and policy.

  • Participate in continuing education as required by the state Board of Nursing

  • Prescribe medication as permitted by the state Board of Nursing

  • Other related duties as deemed necessary


Qualifications of the Advanced Practice Provider

  • Family, Adult Nurse Practitioner or Physician Assistant with appropriate certification and licensure is required

  • Functions to the full level of the NP/PA license as a member of a multidisciplinary care team to provide age-appropriate, high-quality health care as appropriate to the patients receiving clinical management services

  • Multiple state licenses in CA, AL, TN, IN, FL, GA, or IN strongly preferred

  • 2+ years working with adult or geriatric populations

  • Skilled at using online tools and technology to deliver care and communicate with patients

  • Consistently demonstrate clinical competency in the management of chronic diseases for patients with complex comorbidities.

  • Possession of DEA registration or eligibility

  • Upperline’s APPs must have strong compassion for our patients and their families

  • Thrives in a fast-paced, high-stress, demanding medical environment

  • Excellent oral and written communication skills to ensure that each of your interactions with patients, family caregivers, clinicians, and team members is effective and meaningful.

  • Experience in value-based care setting is a plus

  • Spanish fluency - strong plus!



Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
