Description

Position Summary:

Responsible for ensuring that clients receive the individual range of medical care and other comprehensive services necessary to optimize each client's substance use, mental health, and health outcomes.

Essential Job Functions:

  • It is responsible for the social evaluations of all Project participants.

  • Responsible for participating in the initial interview for the determination of needs and maintaining the social reports of each of the participants, carrying out assessments of an emotional nature, self-esteem, independent living capacities, for the different departments needed.

  • Takes care of making the participant's social history.

  • Assess participants' needs in conjunction with the team and others peers

  • In charge of all that is support groups and carrying out service activities on a daily basis. Gives individual and group therapies aimed at mastering social and community skills.

  • It is responsible for providing guidance related to the search for housing and employment services.

  • It contributes to the development and increase of Social Welfare, involved in the individual, group and community, as well as to the institutions and services related to the various Areas of Welfare and Social Services.

  • Coordinates and applies all socio-health resources to the client's reach.

  • It also helps in the integral development of clients and the family in collaboration with the team.

  • Hold self-help meetings, with the participant's family and/or partner.

  • Review of records and compliance with participant eligibility documents, in coordination with Coordinator and Case Managers

  • It keeps the Coordinator aware of situations that occurred during his/her interventions.

  • Represents and accompanies clients to court hearings.

  • Set new goals and goals so that each participant can develop a life plan and reintegrate into the community by implementing the skills learned within the help center.

  • Other tasks to be assigned by the Supervisor.

  • Pursuant to Executive Order 2021-075 you must submit proof of vaccination against COVID-19 and pursuant to Order 2021-082 evidence of vaccine booster; or instead medical or religious exemption that meets the requirements set forth in the executive order, as well as negative PCR test or antigens every 7 days.

Qualifications

Education:

Bachelors or Masters Degree in Social Work, Licensed Social Worker, and active member of the Social Workers Board, 2 to 5 years of Experience, Knowledge and based evidence practices. Computer knowledge.

Experience and Competencies:

  • Bilingual (Spanish/English)

    Working Conditions:

  • Work is primarily performed in an office or residential treatment setting; and

  • Some outdoor activities may be required.
