Job Summary

Assesses, plans, implements and evaluates the nursing care of residents within the skilled nursing setting. Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNA’s, LPN’s and supportive staff members. Participates in resident and family teaching/conferences. Maintains a role as a resident advocate with a focus on the facility’s mission.

Core Job Responsibilities

  • Assesses residents on admission and, as well as when conditions change. Documents appropriately including Care Plans and Medical Record. Educates LPNs on PPS documentation requirements and ensures compliance.

  • Assesses and reassesses pain using appropriate pain management techniques. Educates the resident and family regarding pain management. Reviews all records and ensures pain log is complete.

  • Assesses the resident’s condition and nursing needs; sets goals; prescribes appropriate nursing action.

  • Performs all aspects of resident care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

  • Submits records and written documentation in timely, clear, concise and pertinent manner.

  • Notifies physician of changes in resident’s condition and follows through until appropriate action is taken.

  • Notes and carries out physician’s orders in a timely manner. Accompanies physician on floor rounds and prepares all 30 and 60 day reviews in advance.

  • Gives a thorough report to oncoming shift by participating in walking rounds.

  • Follows through timely on information given by shift report, resident or family regarding resident concerns.

  • Documents complaints on appropriate form. Investigates and develops solutions, gives feedback to resident/family. Keeps leadership informed.

  • Implements plan of care for the resident based on assessments and goals as established by the interdisciplinary care team. Evaluates plan of care for effectiveness and revises as indicated by the resident’s response to treatment.

  • Treats all residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and spiritual needs of residents and their families. Interacts with residents, families, co-workers, managers and other staff in a kind, knowledgeable and friendly manner.

  • Administers medications, following the five medication rights and reduces the potential for medication errors.

  • Identifies and interprets clinical changes as they relate to medical diagnosis.

  • Takes personal responsibility and accountability for professional practice and growth for self, others and the nursing profession. Learns new clinical skills independently.

  • Supervises and leads other members of the healthcare team to accomplish the plan of care which has been developed. Delegates to LPNs and CNAs in a manner that contributes to the excellence of the nursing unit.

  • Acts as a preceptor/resource person and assists others in implementing nursing care.

  • Functions as a resident advocate by protecting the resident’s rights.

  • Remains calm in emergency and/or stressful situations.

  • Complies with all applicable regulations and MVHS policies and procedures.

  • Performs other duties as required.

Education/Experience Requirements

REQUIRED:

  • Graduate of an accredited nursing program

  • Good interpersonal, written and verbal communication skills.

PREFERRED:

  • Bachelor of Science in Nursing from an accredited nursing program or higher degree

  • One year experience in long term care

Licensure/Certification Requirements

REQUIRED:

  • Registered Nurse licensure in NYS.

Preferred:

  • Basic Life Support Certification
