You will become a part of highly regarded and successful community service lines that are an important part of WesleyLife's mission and ministry. You will be asked to continue our reputation of excellence in service, hospitality, and quality as you join a committed group of team members who are passionate about the mission of our organization. As a Wellness Coach, you will provide personalized wellness coaching that helps to ensure members are able to continue the opportunity to pursue their joy while living in their own homes, receiving support and coaching to promote health and well-being. The purpose is holistic well-being, independence, safety and happiness!

A typical day as a Wellness Coach with WellAhead:

Wellness Coaching:

  • Manages a caseload of 30-35 WellAhead program members, providing personalized wellness coaching throughout their program participation.

  • Designs and implements holistic wellness programs to support members in maintaining independence and well-being at home.

  • Collaborates with the Executive Director to ensure timely deployment of services tailored to members' needs.

  • Works closely with the Wellness Coaching team to ensure seamless support for all program members.

Relationship Building:

  • Establishes trusting relationships with members to enhance their well-being, independence, and safety.

  • Maintains confidentiality and acts as an advocate for each member throughout their tenure in the WellAhead program.

  • Encourages healthy living practices and connects members with resources for ongoing support.

Wellness Advocacy:

  • Promotes holistic wellness programming to empower WellAhead members to maintain independence and well-being.

  • Explores innovative approaches to enhance services for program members.

  • Collaborates with members' care teams, including family, friends, community, and health partners.

Documentation and Professionalism:

  • Maintains accurate documentation of member interactions and service needs to support independence and safety at home.

  • Upholds confidentiality while sharing relevant information with the Lead Wellness Coach and/or Executive Director.

  • Demonstrates commitment to professional growth through certifications, training, and collaboration with colleagues.

Competency and Knowledge Requirements:

  • Preferred experience in diverse health and well-being settings, particularly with aging populations.

  • Strong case management skills and willingness to obtain CPR/AED certification.

  • Good general health and emotional stability.

  • Demonstrates a strong customer service attitude and commitment to excellence.

Compensation:

  • The pay range for this position is $25.50 / hour - $32.25 / hour. The starting rate will be based on years of experience.
