Kaiser Permanente Part Time - Clinical Chaplain, Care at Home - Tustin in Tustin, California

Job Summary:

To provide spiritual support and counseling to home care patients, families and staff in a culturally inclusive manner. Home Care is defined as Home Health, Hospice, Home Palliative or any future at home service offerings as prescribed by KP. The Chaplain is a member of the interdisciplinary patient care team, participates in patient care planning and documents all pertinent interventions in the patient care record using established charting methodologies.

Essential Responsibilities:

  • Administrative Support:

  • Participates in Quality Assurance Activities to include but not limited to committees for Patient Satisfaction Scores, QI Projects, UBT, etc. as requested or in collaboration with agency leadership.

  • May serve on Committees and work in collaboration with committees such as Bioethics, EAP, LCP, ELOA.

  • Conducts In-services and CEU programs to improve Integrated Model of Care to include but not limited to New Employee Orientation and Interdisciplinary Meetings.

  • Provides Staff support to maintain Spiritual Resilience in HealthCare to include but not limited to IDG & All Staff reflections, speakers and rituals.

  • Provides faith/ value-based rites/rituals and maintains collaboration with Community Clergy to ensure support to staff, patients and families for religious specific spiritual care needs.

  • Acts as a consult to community liaisons faith leaders regarding serving in a HealthCare setting as appropriate for collaboration in meeting homecare patient needs.

  • Provides spiritual support and spiritual counseling to staff members such as but not limited to, de-briefs, complex grief as requested by leadership.

  • Maintains compliance with the Standards of Practice for Professional Chaplains and demonstrates Common Qualifications and Competencies for Professional Chaplaincy as defined by the APC (Association of Professional Chaplains).

  • Attends education programs to assist in improving skills related to provision of homecare services.

  • Other duties as assigned.

  • Direct Patient Care:

  • Provides spiritual support and counseling to home care patients, families and staff in a culturally competent manner that upholds the Kaiser Permanente Principles of Responsibility.

  • Clinical Chaplain is a member of the interdisciplinary patient care team, using evidence-based outcome-oriented assessment tools to participate in creating patient care plan that includes acuity mapping to define frequency of visits/duration, documents interventions/outcomes and meets the TJC/regulatory guidelines of Hospice/Homecare.

  • Provides spiritual support or visits at the time of death as assessed by patient/family need.

  • Will support the admission / consent process in collaboration with the inter-disciplinary team in the explanation of hospice benefits to the patient and families at leadership request.

  • Supports bereavement care plan to surviving family members for up to 13 months after patient dies; including phone support, in home visitations, bereavement support groups and memorial. Chaplains assist with funeral services for deceased patients when requested and as scheduling allows.

  • Will vary schedule in collaboration with leadership to accommodate need for 24-hour coverage.

Basic Qualifications:

Experience

  • Minimum of one (1) year of experience as a Clinical Chaplain/Spiritual Counselor in a Hospital, Ambulatory, Home Health, Palliative Care or Hospice setting required.

Education

  • Masters Degree in Divinity, Theology or related field from an accredited Seminary, College or University required. Minimum of four (4) units of Clinical Pastoral Education from an ACPE accredited program (ACPE is the CPE program accredited by the U.S. Department of Education).

License, Certification, Registration

  • Board Certified Chaplain Certification from Association of Professional Chaplains OR National Association of Catholic Chaplains Certification OR National Association of Jewish Chaplains Certified Member

  • Driver's License (California)

  • Basic Life Support

Additional Requirements:

  • Must be able to serve & support people of all religions, world views, and spiritual perspectives.

  • Maintains adequate automobile liability insurance as required by Hospice Program policy.

  • Meet Medical Center/Homecare requirements for health appraisal and annual PPD testing.

  • Behaves in a courteous, professional manner with patients, staff and health care professionals and maintains patient confidentiality.

  • Appropriate age specific competencies to be successfully completed by end of orientation: Infant, Toddler, Child, Adolescent, Adult and Geriatric.

  • Conveys a sense of maturity and sensitivity with good listening skills as evident on interviewing.

  • Ability to prioritize and manage multiple tasks and crisis situations.

  • Demonstrated excellent reading, writing and verbal communication skills.

  • Ability to recognize urgent patient needs and how to report it.

  • Must be able to work in a Labor/Management Partnership environment.

Preferred Qualifications:

  • Two (2) or more years of experience as a Clinical Chaplain/ Spiritual Counselor in a Hospital, Home Health, Palliative or Hospice Setting.

  • PhD, DMin, ThD or EdD from an accredited Seminary, College or University. Certificates specializing in Palliative or Hospice Care.
