POSITION SUMMARY

This position, Adult System of Care Coordinator (ASOC) works within the Banner University Health Plan Complete Care program and is responsible for day-to-day activities related to physical, behavioral health, and integrated programs for members 18 years of age and older. Supports adults with, or at risk for, mental health and substance use challenges. This role Ensures programs and resources are organized into a coordinated network, creates meaningful partnerships with individuals served, and address members’ cultural and linguistic needs in order to help the individual identify and achieve personal recovery goals and desired goals at home, in school, in the workplace, in the community, and throughout life.

CORE FUNCTIONS

  1. Ensures coordination of care with system stakeholders and engages in community and provider outreach. May participate in Neighborhood Network Advisory Committees.

  2. Addresses barriers to delivery of healthcare services that are available to all individuals entitled to Title XIX funding, including General Mental Health/Substance Use members as well as individuals determined to have a Serious Mental Illness (SMI).

  3. Ensures compliance with policies and regulations pertaining to the provision of behavioral health services to individuals identified as having a Serious Mental Illness and ensures individuals who receive behavioral health services through Non Title XIX funding receive behavioral health services as described by the Non-Discretionary Federal Grants policy.

  4. Ensures adherence to ASOC guiding principles to promote recovery in the adult behavioral health program, development initiatives, program development, service provision, and stakeholder collaboration.

  5. Tracks and report on key outcome indicators and other regulatory deliverables related to adult healthcare. Works with Neighborhood Networks and the Quality Management department to monitor mechanisms for various evidence-based practices related to the delivery of effective whole person care.

  6. Identifies and responds to community health needs while implementing strategies that positively impact social determinants of health. Responds to the needs of members with special healthcare needs and/or chronic health conditions, partners with Centers of Excellence, and the development of incubators to drive innovations – especially related to opioid addiction, families that are DCS involved, homeless populations, and justice involved adults.

  7. Provides guidance and consultation related to specific member needs.

  8. Facility and community based. Internal customers: BUHP SOC leaders and team members, and other departments within the BUHP. External customers: AHCCCS, Families, Members, community agencies, juvenile justice partners, Department of Child Safety, advocacy groups, and providers (Neighborhood Networks). Must make decisions independently in line with established guidelines and procedures.

MINIMUM QUALIFICATIONS

Knowledge, skills and abilities as normally obtained through the completion of a Bachelor’s degree in Social Work, Counseling, Behavioral Health or other Healthcare related field.

3 years of experience in Healthcare, Case/Care Management, Clinical Management, Utilization, Quality management or Medical Management.

PREFERRED QUALIFICATIONS

Experience in Arizona Medicaid environment.

Additional related education and/or experience preferred.
