Requisition: 2024-137205 TEAM COORDINATOR Job Locations US-OH-TROY ID 2024-137205 Line of Business Ohio Valley Hospice Position Type Full-Time Our Company Ohio Valley Hospice Overview The Team Coordinator synchronizes the daily affairs of the team's needs: daily census, level of care changes, schedules, assignments, medical records, supply ordering, stubbing in visits, pre-billing reports, general office duties/support and team communication. The team coordinator's intention is to provide support and help to the ADON, and each discipline within the team. The team coordinator provides direction and organization for the team. Responsibilities * Assist the Assistant Director of Nursing (ADON), and the Patient Care Staff * Maintain daily census, tracking admissions, deaths, discharges and level of care changes * Notification to team of patient discharge or change of status * Maintaining medical records, which includes the scanning of medical records documents into the Electronic Medical Record system (EMR) * Review legal paperwork, and EMR for all admissions * Track paperwork for all discharges (ie: death certificate, death pronouncement, revocation form, transfer form) ensure signatures obtained, providing yellow copy of death pronouncement to Unit Secretary, & scanning into EMR system * Obtaining field staff schedules each Friday, and ensuring all patients are assigned in EMR * Team & staff assignments of all new patients in the EMR, and communicating assignment to the appropriate team member * Tracking of Death certificates, obtaining signature and reporting back to the GAVERS system * Stubbing of visits that are completed on paper into the EMR system * Arrange transport for patients that will be coming or leaving the IPU * Back-up for covering phones, also covering the unit secretary during lunch, breaks and absences * Prepare for IDT, copy/fax IDT notes to attending physicians, tracking recertification periods and alert ADON and Patient Care Manager (PCM) of upcoming recertification * Participate in staff meetings, team meetings, in-services and other related activities as needed * Abides by and demonstrates the company Mission-Vision-Values through both behavior and job performance on a day-to-day basics * Adheres to and participates in company mandatory HIPPA privacy program/practices and Business Ethics and Compliance programs/practices * Participates in special projects and performs other duties as assigned Qualifications * Must have previous clerical experience-healthcare preferred * Must have reliable transportation and maintain a current driver license in the state and maintain automobile liability insurance. * Must be able to understand, read, write and speak English. * Must be able to relate positively and favorably to the volunteers, patients, families and outside agencies and to work cooperatively with other employees. * Must understand and accept hospice philosophy. * Must demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities. * Must comply with acceptable professional standards and practice.
