We are currently hiring a Case Manager - Troy , to work in our Housing Office. ? As the Case Manager , you would be responsible for: ?
Facilitate the transition of homeless persons who enter the program and assist them in maintaining permanent housing and support services
Perform assessments
Develop and implement Individualized Service Plans
Provide support and supervision to residents during evening hours
Organizes and facilitates support groups and social and recreation activities
Case Manager Minimum Salary: $18.00 per hour
Case Manager Maximum Salary: $20.00 per hour
Case Manager Schedule: Monday – Friday, 2:00pm – 10:00pm
Qualifications:
Associates Degree (preferred)
1-3 years of experience
Valid Drivers License
Experience working with people experiencing homelessness
Experience working with diverse populations
