As a Physical Therapist with AdvisaCare, you will provide quality care in patients' homes, helping them to regain independence and improve their quality of life. Responsibilities
  • Conduct initial and ongoing patient assessments to determine individualized treatment plans and goals.
  • Develop and implement physical therapy interventions based on best practice guidelines, patient needs, and physician orders.
  • Monitor and document patient progress and modify treatment plans as needed.
  • Educate patients and caregivers on exercises and activities to improve mobility, strength, balance, and coordination.
  • Collaborate with interdisciplinary team members to ensure delivery of quality, holistic care.
  • Adhere to all legal, ethical, and regulatory guidelines and standards.

Requirements {#job-requirements-title}
  • Valid and current Physical Therapist license in the state of Michigan.
  • Minimum of one year of experience as a Physical Therapist, including home health experience, is preferred.
  • Strong clinical skills, including assessment, treatment planning, and implementation.
  • Ability to work independently and as part of a team, with excellent communication and collaboration skills.
  • Flexible and adaptable, with a positive attitude and customer-focused approach.
  • Ability to navigate technology and electronic medical records.
