Ministry Support (Diakon Lutheran Social Ministries) Case Manager II in Topton, Pennsylvania

Category/Shift/Hours per 2 week pay period:  Full-Time / Varied Shift / 75 hours per pay period

Diakon Lutheran Social Ministries is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Senior Living Services and Diakon Child, Family, and Community Ministries.

 

General Responsibilities:

  • Responsible for serving as a resource person and case manager to support families involved in the Permanency Services program.  
  • Establishes and maintains professional and supportive relationships with youth, resource families, and birth families, referring personnel, treatment providers, and others involved in the lives of children and youth.
  • Meets regularly in the youth's home to provide skill development and on-going case management services to coordinate the treatment planning and services provided according to the Treatment Plan and clinical direction. 
  • Maintains regular contact with referring personnel and attends and testifies at court proceedings involving children and youth in care.
  • Childline & DHS FBI clearance results are required prior to beginning employment.

     

Education/Training: A bachelor's degree in social service or a related field of study normally required; A Master's Degree preferred. 

 

Experience: A minimum of two (2) year's related experience normally required.

 

ADA: Ability to sit, stand, bend, stoop, reach, and lift items weighing up to 50 lbs.; visual and auditory skills.
