Summary: Under the Supervision of the Director of Direct Services provide Intensive Case Management as directed to ensure wellbeing, stability, health, safety and avenues for self-sufficiency for Ocean Inc.s clients in the following areas: Housing- stabilization of adequate and affordable housing. Employment-assessing employable skills and abilities. Income- identifying and assist in obtaining eligibility for incomebased programs. Child Care-assist in obtaining quality childcare to enable parents to work or attend training. Mobility-assist in locating options for transportation. Education-direct towards literacy and internet education. Health Care-provide information on current program available Essential Duties & Responsibilities The Intensive Case Management Services involves outreach to clients to provide adequate and affordable housing and to meet basic needs to help the client achieve self-sufficiency. It consists at the least mandatory weekly face to face meetings with recipients and home visits to the clients current residence to determine what services may be needed. All potential options should be explored with the recipients. Assist with the obtaining and completion of documents, applications, and records needed on the behalf of the client. Transport individuals to various locations in the community to enable shelter and basic needs are met. Develop and maintain working relationship with other agencies including Mental Health facilities, Public Defenders, Social Security Office, DYFS, Boards of Social Services, landlords, HUD low income housing list and other agencies. Documenting interventions and outcomes for each referral which will include at a minimum, all outreach efforts to engage the recipient, any specific accommodations for each individual, progress reports of each meeting, listing of all options offered to the recipient, the outcome of each option explored or result of referrals and the final disposition of each case. Input case notes and expenditures information into the Homeless Management Information System and Empower. Other duties as assigned that are deemed necessary for program operation. Attends staff / department meetings, trainings and workshops as assigned Other Duties Qualifications BA/BS in Social Work or related filed and/or experience in case management. Experience working with local social service agencies, seniors, mentally ill, disabled, homeless a plus Solid organizational skills Proficient in all Microsoft applications and ability to learn MINC, NJHMIS and other computer programs as required. Ability to coordinate and perform multiple tasks as simultaneously Ability to relate well with others and to respond to the needs of tenants in a timely and efficient manner Ability to work outside of normal business hours when necessary Required to speak, read, and write English and Spanish (preferred not required) Required to deal effectively with public and staff Required to communicate and work with adults from diverse backgrounds Valid driver license and access to transportation as needed Physical Demands Visual and auditory acuity within professionally determined normal ranges, with correction if needed.
