SUMMARY: Responsible for providing comprehensive needs assessments, planning, advocacy, and follow-up for students and families in need of behavioral health care, substance abuse treatment, and/or community resources, in addition to collaborating with community-based organizations to coordinate behavioral health care and substance abuse trainings and workshops for school staff. This position will also assist families in a culturally responsive manner with understanding the benefits as well as the responsibilities of behavioral health care and/or substance abuse treatment; work with school staff to ensure an understanding of communicating and working with parents/guardians of students with behavioral needs or substance abuse concerns; and directly support students and families with intakes at community-based organizations as well as Medicaid and CHP+ enrollment.

EDUCATION AND RELATED WORK EXPERIENCE:

  • Masters Degree in related area.

  • Experience working with students in a school environment required.

  • Bilingual Spanish

LICENSES, REGISTRATIONS or CERTIFICATIONS:

  • Criminal background check required for hire.

  • Must possess or be able to obtain a Special Services Provider License with the Colorado Department of Education.

TECHNICAL SKILLS, KNOWLEDGE & ABILITIES:

  • Knowledge of mental health diagnoses and behavior disorders.

  • Ability to collaborate closely with a team of family members and professionals to develop and implement individualized plans of care

  • Knowledge of community-based resources available to students and families. Ability to create, establish and maintain relationships with those agencies.

  • Ability to work a flexible schedule, must be able to meet with student and family teams during times and dates that are convenient for them.

  • Provide and coordinate services to students and families that maintain and strengthen the relationship between student, family and the school.

  • Meet face-to-face with families in their own home for provision of wraparound services

  • Maintain an empathetic, yet professional, relationship with the families serviced and strive to keep families engaged in the wraparound process and the program.

  • Assist in the development and implementation of an individualized wraparound plan for each student that includes clear need statements, strength-based strategies, and outcomes that are measurable and specific.

  • Active participant in data collection, analysis and presentation of the impact of the services to the family, school staff and IEP teams.

  • Identify, coordinate and participate in IEP meetings for students and student progress meetings.

  • Keep the family, school staff and team members informed on progress toward achieving the goals of the wraparound plan.

  • Facilitate student and family efforts to obtain any financial assistance or services for which they may be eligible under federal, state and local regulation.

  • Maintain accurate and concise documentation of contacts with family, team members and providers, including follow-up on referrals made.

  • Deliver services and supports that reflect the unique cultural values and practices of each student and family.

  • Ability to promote and follow Board of Education policies, District Policies, building and department procedures.

  • Ability to communicate, interact and work effectively and cooperatively with all people, including those from diverse ethnic and educational backgrounds. Willingness to contribute to cultural diversity for educational enrichment.

  • Ability to recognize the importance of safety in the workplace, follow safety rules, practice safe work habits, utilize appropriate safety equipment and report unsafe conditions to the appropriate administrator.

  • Ability to stay current with district policy, standards and training in the areas of data quality, data privacy, and cybersecurity with respect to student and staff data, and related information systems.

  • SALARY INFORMATION (FOR EXTERNAL CANDIDATES):

  • Salary placement on ourCertified Salary schedule (https://www.adams12.org/our-district/careers/teach) is dependent on the employees’ education and/or work experience

  • Certified/licensed employees will be granted one (1) year for each full year of contractual and/or work experience up to a maximum of ten (10) years, which includes in-district and out-of-district contractual teaching and/or work experience. Responsibility for verification of outside experience shall rest with the individual employee.

Internal Candidates Only:

13.2.5 Voluntary transfers that occur fifteen working days before the start of the school year and until the end of the school year shall happen once the position being vacated has been filled, for a duration of no more than four weeks. This four week time period could be adjusted in consultation with the educator, current and receiving supervisor and with approval from the Chief Human Resources Officer and the Association President.
