Errinrung Retirement Home in Thornbury, ON is looking for a Wellness Coordinator to join their team. This position will be a full time, permanent role at Errinrung.
On a daily basis, this role will involve tasks such as:
Collaborates with Management team in overseeing nursing department activities (PSW, DSW, UCP's)
Reporting and adhering to RHRA policies and procedures
Conducting consultations with residents and their families
Develop, discuss and revise Care Plans for residents
Completing Physical Assessments
Reviewing lab work, Intake forms, and referrals
Assisting with doctor appointments
Health Education and Teaching (staff and residents)
Processing Physician orders
Leads the implementation and monitoring of required clinical programs, i.e. Fall Prevention, Infection Control, Skin and Wound Care, Continence Care, Pain Management
Coordinates with community resources and supports
Actively participates in staff and resident committee meetings
Performs other duties as needed or required
A successful candidate for this position will:
Be an experienced leader with ability to coach and mentor team members
Works effectively with an interdisciplinary team
Valid registration with the CNO (Registered Practical Nurse/Registered Nurse)
Experience in a Retirement setting is an asset
Police Record/Vulnerable Sectors Check
Ability to problem solve Advanced organizational skills
