Position Summary:

The Nurse Practitioner in an outpatient nephrology clinic plays a crucial role in providing comprehensive care to patients with kidney-related conditions( mainly Chronic Kidney disease related). Working closely with nephrologists and other healthcare professionals, the NP is responsible for assessing, diagnosing, and managing the care of patients with various kidney diseases. The focus is on delivering high-quality, patient-centered care while collaborating with the multidisciplinary healthcare team.

Position Key Accountabilities:

  1. Patient Assessment: Conduct thorough assessments of patients with kidney-related concerns, including medical history, physical examinations, and diagnostic tests.

  2. Diagnosis and Treatment: Formulate accurate diagnoses based on assessment findings and collaborate with the healthcare team to develop and implement appropriate treatment plans.

  3. Medication Management: Prescribe medications as needed, monitor patient responses, and adjust treatment plans accordingly. Educate patients on medication management and potential side effects.

  4. Patient Education: Provide comprehensive education to patients and their families on kidney disease management, lifestyle modifications, and preventive measures.

  5. Collaboration: Collaborate with nephrologists, nurses, dietitians, and other healthcare professionals.

  6. Follow-up Care: Conduct regular follow-up appointments to monitor patient progress, adjust treatment plans, and address any concerns or complications.

  7. Documentation: Maintain accurate and up-to-date electronic health records (EHR) and documentation of patient assessments, diagnoses, treatments, and education provided.

  8. Quality Improvement: Participate in quality improvement initiatives to enhance the delivery of care, patient outcomes, and clinic efficiency.

Certification/Skills:

Current Texas Licensure as a Registered Nurse

Current Texas Licensure as an APRN-NP, and specialty certification as required by department

Basic Life Support (BLS) Certified or specialty certification as required by department

Minimum Education:

Masters in Nursing as NP or Masters in Nursing with completion of a post masters Nurse Practitioner Certification Program.

Minimum Experience:

None

Physical Requirements:

Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
