JOB DESCRIPTION
Personal Care Aide

JOB SUMMARY:

Responsible for assisting CNHHS members with essential daily living needs such as feeding, personal care, running errands, and routine housekeeping in their homes and in accordance with their plan of care.

QUALIFICATIONS:

1. Must be at least 18 years of age.

2. Have valid drivers license, auto insurance, and dependable transportation.

3. Must have a smartphone or tablet to install the State required app for clocking in/out.

4. Have a sympathetic attitude toward the care of the disabled and elderly.

5. Ability to carry out directions, read and write.

6. Maturity and ability to deal effectively with the demands of the job.

RESPONSIBILITIES:

1. Understands and adheres to established CNHHS policies and procedures.

2. Follow each members plan of care as directed, performing duties while member is present.

3. Assist with personal care: dressing/grooming/bathing/toileting/hygiene.

4. Prepare meals, clean kitchen, and wash dishes.

5. Perform light housekeeping: (floors, dusting, counters, bathrooms, trash.)

6. Run errands: (pick up groceries/supply/medication/pay utility bills)

7. Wash and dry laundry in members home or laundromat.

8. Perform in-home respite and/or Advanced Supportive/Restorative care as assigned.

9. Immediately inform Staffing Coordinator for a missed visits or changes in assigned schedule. Missed visits may only be made up when pre-approved by the Staffing Coordinator. PCA must follow schedule as assigned to ensure presents when RN arrives to complete their SUP visit as required.

10. Report changes in members condition or status updates to Staffing Coordinator.

11. Complete a minimum of 12 hours of education annually by attending all mandatory in-service meetings or other educational assignments.

12. Follow agency and State guidelines for clocking in/out. Failure to notify Staffing Coordinator of time corrections by agency deadline may result in not being paid timely.

WORKING ENVIRONMENT:

Works indoors in members homes and driving in various climates.

1. Supervised by:Staffing Coordinator(s)

RISK EXPOSURE:

High risk. Exposure to household chemicals.

LIFTING REQUIREMENTS:

Ability to perform the following tasks if necessary:

Ability to work for extended period of time while standing and being involved in physical activity.

Must be able to lift a minimum of 25 lbs.

Ability to do extensive bending, lifting and standing on a regular basis.
