Crouse Hospital Senior Population Health Registered Nurse: Crouse Medical Practice in Syracuse, New York

This position works for Crouse Medical Practice , a multi-specialty physician practice with several locations to serve healthcare needs in the Syracuse, NY community!

JOB SUMMARY:

The Senior Population Health Nurse will serve as a clinical leader in the Population Health and Quality Improvement department, mentoring colleague nurses in assessment and intervention techniques, providing guidance on complex or difficult cases, advising leadership as new clinical programming is developed, and representing the department on improvement teams. The Senior Population Health Nurse will also function as a nurse care coordinator and coordinate all systems/services required for an organized, multidisciplinary, patient centered care team approach, and assure quality, cost- effective care for the identified patient population. Manage the course of treatment for patients by coordinating care with physicians, nurses, other staff and community health care providers, ensuring quality patient outcomes are achieved within established time frames and with efficient utilization of resources. S/he interfaces with community providers, insurance companies and agencies and providers along the care continuum. Direct the work of LPN Care Coordinators. Participate in Patient Centered Medical Home activities. This is a remote position. The successful candidate must live in Central New York.

MINIMUM REQUIREMENTS:

  • EDUCATION/CERTIFICATION/LICENSURE:

  • Certification in Case Management (CCM, ACM) preferred

  • Current RN Licensure in New York; Associates Degree in Nursing, Bachelor’s Degree in Nursing (BSN) preferred

  • EXPERIENCE:

  • REQUIRED:

  • Self-disciplined, energetic, passionate, and innovative

  • Confidence to communicate and outreach to other community health care organizations and personnel

  • Able to maintain confidentiality with all aspects of information in accordance with Practice, State and Federal regulations

  • Demonstrates sound judgment, decision-making, and problem-solving skills

  • Highly organized and well-developed oral and written skills

  • Comprehensive knowledge of insurance company principles and outpatient delivery systems

  • Knowledge of provider community and community resources in the Syracuse NY/CNY area

  • Experience in program and process assessment, and improvement.

  • Experience in mentoring nurses and other clinical staff.

  • Leadership experience assessing strengths an opportunity for nurses and other clinical staff

  • Experience in coordinating care for patients across the care continuum, including home-based, primary and specialty care and long term care.

  • PREFERRED:

  • Minimum 5 years clinical experience

  • Experience working in primary and long term care with adult patients

  • SKILLS/PROFICIENCIES:

  • Excellent communication and teaching skills

  • Self-disciplined, energetic, passionate and innovative

  • Demonstrated commitment to collaboration with providers and their clinical teams to enhance patient self-management

  • Diligent attention to accurate recording of patient information

    Pay Range: $59,000.00 - $73,000.00 DOE
