Work Location: YMCA Downtown Branch, Syracuse, NY

Shift Schedule: Varies

Salary Range: $21.00 - $25.00/hour

Position Summary:

This position supports the mission and work of the Y, a leading nonprofit, charitable organization. Under the direction of the Director of Case Management & Programming, the Residence Case Manager (RCM) will provide individual case management to help residents transition into self-sufficiency and independent living. The RCM will work 1-on-1 with residents to develop specific housing goal plans and provide mentoring in accomplishment of personal objectives. The RCM assures the well-being for each participant in line with YMCA of the USA guidelines and association policies in accordance with their training.

Salary and Benefits:

  • $21.00-$25.00/hour, Full Time- Non-exempt

    Essential Functions:

  • Models the YMCA core values of caring, honesty, respect, and responsibility with residents, members, colleagues, guests, volunteers, and vendors.

  • Follows all Residence procedures and guidelines including those pertaining to health and safety, sessions, forms, reports, and staff expectations.

  • Knows, follows, and enforces all YMCA policies, rules, regulations, and procedures, including emergency and safety procedures, and those for the prevention of child abuse.

  • Attends all mandatory meetings and trainings.

  • Review all referrals to the Residence Department and conduct intake interviews for potential residents.

  • Work with the RCS to review resident information and assess the residents’ current service plan.

  • Make service recommendations to enhance the residents’ goal plan and quality of life.

  • Conduct 1-on-1 sessions with current residents to provide access to support and treatment programs.

  • Maintains proper individual file documentation and case notes, and prepares required reports as directed. Responsible for management of case notes assignments to all department personnel.

  • Provide ongoing support, guidance, and direction which will enhance a congregate residence environment that promotes individual success.

  • Assist in determining if the YMCA is an appropriate housing option for each potential resident and ensure continuous support is in place for lasting results.

  • Directly supervise Resident Advisors and ensure the quality of their performance.

  • Obtain necessary records from hospitals, outpatient providers, vocational, housing, HMIS, and other service areas pertaining to individual clients; make referrals of clients to necessary outside services.

  • Assist residents in obtaining permanent housing, employment, mental health, and other means of stability; provide follow up services.

  • Act as Agency Administrator for HMIS database, including oversight of data entry and attendance in HMIS related meetings and conferences.

  • Maintain professionalism and positive representation of the program at all times.

  • Act as vital and contributing member of the Y management team; participate in the Manager-on-duty (MOD) on-call rotation.

    Experience, Education, and Qualifications:

  • Bachelor's Degree in Social Work, Human Services or related field- required. Masters Degree in Social Work, Human Services, or related field- preferred. In certain cases, significant relevant experience may be substituted.

  • Previous case management or related experience working directly with the homeless population or other vulnerable populations facing similar challenges. Experience with clients who Identify as men with multiple diagnoses- preferred.

  • CASAC certification- preferred.

  • Experience with HMIS (Homeless Management Information System) - preferred

  • Excellent communication skills and technologically proficient.

  • Experience with motivational interviewing and person centered planning- preferred.

  • Valid driver's license, and reliable transportation, as some travel may be required.

  • Ability to work a flexible schedule, if needed.

  • Bi-lingual in Spanish- preferred.

  • Must be able to lift up to 50 pounds at a time.

  • Will be exposed to illness-causing bacteria and viruses.

    Work Environment:

    This job operates in a recreational and/or educational environment. This role routinely uses standard recreation and/or office equipment. At times, employees may be exposed to undesirable working conditions, communicable infectious diseases, and risk of injury from others. All employees are required to always follow the preventative health policies of the YMCA. The noise level in the work environment is moderate to high.
