SUNY Upstate Medical University Nurse Practitioner/Physician Assistant-Diabetes Center in Syracuse, New York

Job Summary:

The Nurse Practitioner/Physician Assistant will serve as a Diabetes and Endocrine care provider prepared to deliver comprehensive patient care to adult patients. Will assess, diagnose and develop a plan of care for patients in accordance with predetermined joint practice protocols.

Advance Practice Providers evaluate, diagnose, formulate and implement treatment plans, educate and counsel patients and families, prescribe, and perform other responsibilities within the scope of practice of and as delegated by the supervising physician and are committed members of the multi disciplinary health care team that maintain professional competencies while providing high quality, cost effective, patient centered care consistent with the mission, vision and values of Upstate Medical University.

Minimum Qualifications:

NYS licensed/registered NP or PA required. Master’s Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).

CPR-BLS Certification required at the time of appointment.

Preferred Qualifications:

Prior experience in specialty area preferred.
