Position Summary Information

Job Description Summary

Provide care in the School Based Health Centers or at any of our sites. Responsible for providing primary health care to patients of the Center in assigned specialty area; prepare health histories, perform physical examinations, perform selected diagnostic and therapeutic procedures, treatments and laboratory examinations; recommend patient medical management, and internal and external referrals; patient teaching; make hospital calls; participate in on-call rotation system.

Minimum Qualifications

Graduation from an accredited school. NYS license required – Nurse Practitioner or Physician Assistant. Board Certified. Current Basic Life Support BLS certification.

All staff working within the School Based Program must be fingerprinted and receive successful clearance through the New York State Department of Education who will complete criminal background checks.

Bilingual skills preferred.

Physical Demands

Ambulatory – 95%

Sedentary- 5%

Subject to stress and Fatigue

Requires manual and finger dexterity and eye hand coordination when working with patients and equipment.

Requires corrected vision and hearing within normal range
