SUNY Upstate Medical University Neurosurgery Surgical Nurse Navigator in Syracuse, New York

Job Summary:

The Neurosurgery Surgical Nurse Navigator will provide a team-based care working with clinical and administrative staff. The Neurosurgery Surgical Nurse Navigator will guide, support, and educate the patient through their continuum of care with a patient centered focus.

  1. Obtains and documents health history, information on chief complaint, and vital signs. Provides patient care based on practice guidelines, standards of care, and federal/state laws and regulations.

  2. Schedules appropriate procedures/surgeries per providers order and knowledge of operating room scheduling policies and procedures. Will need to coordinate co-surgeries with multiple providers and surgical assistants as necessary.

  3. Utilizes clinical knowledge to review all pretesting results. Notify primary care, surgeon, and care team of any abnormal results. Ensures proper follow up testing is ordered and completed as necessary.

  4. Calls post-operative patients to determine assess their pain level and status.

  5. Executes prescribed treatments and medical interventions, and monitors and documents treatment progress and patient response. Refers to/consults with physicians, other health providers to prevent/resolve problems or concerns. Educates patients and families about health status, health maintenance, and management of acute and chronic conditions.

  6. Obtains insurance authorizations for medical procedures and completes medical disability forms.

  7. Participates in multidisciplinary teams including clinical and administrative staff to improve patient care processes and outcomes.

  8. Answers patient questions via telephone and EMR, triages patient signs and symptoms as necessary.

  9. Serves as a liaison between the surgeon and the patients.

    Minimum Qualifications:

NYS licensed/registered RN. Prior experience in an outpatient setting (specialty area) strongly preferred. Current BLS certification through the American Heart Association or the American Red Cross is required.

Education: minimum requirement AAS RN. Completion of nursing training from an accredited school of nursing.

Preferred Qualifications:

Minimum three years of professional nursing experience.

Work Days:

Monday -Friday 8:00am -4:30pm in physician office setting
