SUNY Upstate Medical University Clinical Case Manager in Syracuse, New York

Job Summary:

The Clinical Case Manager's primary responsibility is to facilitate patient care across the continuum of an assigned number of cases by managing, coordinating, facilitating and monitoring resources to achieve optimal clinical outcomes, utilization of services and financial goals. Oversees the discharge process for all patients within the unit/core service area. The Case Manager is integral to the team effort to establish a safe and appropriate discharge plan/plan of care. It is essential that the case manager possess a working knowledge of the spectrum of hospital and post discharge services that are available.

Minimum Qualifications:

NYS licensed RN (or eligible), Bachelor's degree in Nursing or a healthcare-related field, plus a minimum of three (3) years acute care RN or acute care, transitional care, or chronic care RN/case management experience OR Associates degree in Nursing and five (5) years acute care RN or acute care, transitional care, or chronic care RN/case management experience required. *If Associate in Nursing, must be enrolled in an accredited BSN program with an anticipated graduation date within the next 2 years. Will be appointed temporary until receipt of BSN. CPR-BLS certification is required by the time of appointment.

Preferred Qualifications:

Master's degree in Nursing or healthcare related field, prior acute care or community case management experience preferred. PRI certification a plus.

Work Days:

Monday-Friday, every other weekends, with some holiday coverage
