Hospice Of San Joaquin Chaplain/Spiritual Counselor - Part Time in Stockton, California

Hospice of San Joaquin is the oldest not-for-profit hospice of 40 years.  We serve all of San Joaquin and Stanislaus Counties.  We are committed to continuing to be the leader in ensuring the quality of the journey, from serious illness through end of life, and to honor our mission by honoring life by ensuring patients and their Caregivers are the focus of everything we do.  We are seeking a Dynamic, self-motivated Chaplain who thrives in a fast-paced environment and to support our mission.  This is a Part-Time position with a flexible schedule.

 

POSITION PURPOSE/SUMMARY: Serves the spiritual needs of the Hospice patients, families, staff, and volunteers through direct service and through involving and mobilizing area clergy and lay religious leaders to assist Hospice of San Joaquin in meeting these needs. Provides bereavement support to hospice families and the community.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES/ACCOUNTABILITY

SPIRITUAL - Ensures that patient/family spiritual needs are met and assists the IDT members with spiritual needs and issues.

  • Assess and/or facilitate team assessment of patient/family spiritual needs as related to the terminal illness process.

  • Completes Spiritual Assessment for each patient/family requesting spiritual counseling within 5 working days of request.

  • Maintains a roster of community religious organizations in cooperation with other Hospice team members.

  • Integrates spiritual issues into the plan of care, evaluates the effectiveness of the resource intervention, and initiates follow-up action as needed.

  • Provides consultation and education to interdisciplinary team members and others on spiritual issues and needs of patients, families, and caregivers during the terminal disease process in cooperation with other team members.

  • Advises the IDT regarding community religious organizations and qualified religious representatives available as resources.

  • Contact community clergy at the request of the patient/family.

     

DIRECT SERVICES - Ensures that the spiritual concerns of patients/families, staff, and volunteers are addressed.  If necessary, provides direct services and if possible, requests and encourages the patient’s clergy to visit them.

  • Contribute to the plan of care.                                                                       

  • Provides support and assistance for the spiritual care of the patient/family/caregiver.

  • Performs or assists in obtaining community resources, religious rituals, and funeral services as requested by the patient/family. 

  • Ensures the patient’s/family’s right to confidentiality in all communications.

     

BEREAVEMENT CARE - Provides bereavement care and support to meet the needs of Hospice families and community members.

  • Assess Hospice families and community clients for the needed level of bereavement services.

  • Develop a bereavement plan of care based on own assessments with the collaboration of the Bereavement Services Manager

  • Facilitates regularly scheduled bereavement support groups.

  • Provides goal-directed support and counsel and/or appropriate referrals.

  • Provide ongoing bereavement support to Hospice families per Bereavement protocol.

     

INTERDISCIPLINARY TEAM/COMMUNICATION - Ensures continuity of care to patients/families by effectively collaborating and communicating with IDT members and the patient/family/caregivers.

 

JOB QUALIFICATIONS

  • Pastoral Education degree or comparable training and, or experience.

  • Ability to manage, direct, motivate, and develop human resources.

  • Must possess excellent interpersonal, oral and written skills.

  • Must be able to communicate with people of all religious/spiritual backgrounds without enforcing personal views.

  • Knowledge of the process of terminal illness and the Hospice concept.

  • Skills in clinical assessment and ability to participate in patient care planning.

  • Computer literacy.

  • Must be able to work flexible hours including evenings and weekends.

  • CPR Training

     

OTHER QUALIFICATIONS

  • Must be willing to drive their vehicle. Have a valid California driver’s license and automobile insurance which meets State standards.  Volunteers and/or staff who drive need to provide Hospice of San Joaquin with evidence of insurance.  This is necessary to be covered by the agency’s non-owned auto policy.

  • Able to sit for long periods for visiting with patients/families.

  • Physically able to maneuver throughout the community.

  • Visual / hearing ability sufficient to comprehend written/verbal information.

  • Able to tolerate driving in a car for long periods.

  • Able to negotiate steps and non-paved surfaces to reach patients.
