Job Description

Responsible for nutritional care of patients in assigned units. Plans, assesses, implements, and evaluates patient nutritional care plan. Assists in menu modifications and other nutritional related functions.

Job Responsibility

1.May mentor and train team members.2.Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients' medical record.3..Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration.4.Recommends therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician's order.5.Provides nutritional guidance, education and counseling to patients and their families in accordance with the prescribed diet. Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians.6.Contributes to changes to patient's menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handing and portion control.7.Prepares, presents, and conducts individual and group education classes, and refers clients to community services, as needed. Participates in and promotes Northwell wellness initiatives.8.Develops and utilizes effective lines of communication regarding nutrition information within the program and community.9.Provides in-services to culinary, food service and interdisciplinary teams regarding therapeutic diets and related topics.10.Participates in Quality Assurance/Performance Improvement related tasks including data collection and executes tasks related to it, as directed.11.Performs related duties as required. Job Qualification

Bachelor's Degree or equivalent combination of education and related experience, required.

Current Registered Dietitian Certification with the Commissions on Dietetic Registration, required.

Certified Dietitian Nutritionist in New York State (CDN), as needed.

Advanced/specialized certifications as needed.

1-3 years of relevant experience, required.

The salary range for this position is $58,540-$92,650/year
