HealthPartners Nurse Practitioner - Community Senior Care in St. Louis Park, Minnesota

Park Nicollet Community Senior Care is looking for a fulltime (.8 FTE) Geriatric Nurse Practitioner to join our team. Being a part of our team means you will have an impact on the care that our patients receive every day. As a Park Nicollet Community Senior Care clinician, you will work collaboratively to provide care to an aging population in various long-term care, TCU, assisted living facilities and complex care/home-based medicine. The position is M-F 8am-5pm (no weekends). Voluntary after-hours call. Candidates must possess a nurse practitioner master's degree or DNP; geriatric, adult, or family certification; and MN nursing license. Geriatric experience preferred.
