Northern Counties Health Care is seeking a compassionate and skilled Registered Nurse to join our team to provide triage responsibilities to our St. Johnsbury Community Health Center and Danville Health Center. This position would initially be on-site during training with the potential transition to entirely remote. As a Triage Nurse, the success candidate will be responsible for assessing patients\' conditions over the phone, responding to requests for medical treatment, determining illness acuity based on symptoms, reviewing and refilling medications, and other patient health-related concerns. This role requires excellent communication skills, clinical judgment, and the ability to remain calm under pressure.

Key Responsibilities:

  • Conducting initial assessments of patients\' symptoms and medical history.
  • Providing medical advice and guidance to patients over the phone or through virtual platforms.
  • Prioritizing patient needs based on the severity of their condition.
