MEDICAL CASE MANAGER (Masters Level)

Springfield, Missouri

Summary:

As a Medical Case Manager, you will play a vital role in supporting clients with HIV/AIDS by developing and implementing individualized treatment plans, connecting them with essential resources, and advocating on their behalf. You will work closely with clients, healthcare providers, and community organizations to ensure that our clients receive comprehensive care and support.

Key Responsibilities:

  • Conduct ongoing assessments of clients' personal strengths, progress, problems, and needs

  • Provide individualized assistance to clients in accessing financial, medical, and housing resources

  • Develop and implement critical intervention plans to address clients' unique challenges

  • Maintain regular contact with hospitalized clients and coordinate their care with healthcare providers

  • Complete all required charting and documentation in accordance with operational guidelines

  • Meet service and documentation requirements for various programs, including Ryan White and Greene County Health Department

  • Assist clients in adhering to prescribed medication regimens

  • Facilitate access to mental health care for clients in need

  • Maintain accurate and up-to-date records subject to MDHSS review and inspection

Qualifications:

  • Master's Degree in Social Work, Nursing, or a related field.

  • Demonstrated experience working with adults with HIV/AIDS

  • Reliable transportation and clean driving record

  • Valid driver's license and auto insurance

Work Environment:

You'll be working in the community, visiting clients in their homes, clinics, and other locations. You'll also collaborate with healthcare providers, community organizations, and other stakeholders to ensure that our clients receive seamless, comprehensive care.
