Come help lead our Care Management team at Mercy Medical Center, a member of Trinity Health Of New England!

Position Purpose

The Director of Care Coordination (RN) will direct, plan and lead all activities in our Care Coordination Department. Provide patient-centered leadership while ensuring effective utilization management, compliance with Medicare requirements, and the development of initiatives to reduce readmissions.

What you will do

  • Brainstorm with colleagues to design and implement programs that monitor the delivery of patient care to ensure optimal quality, cost-effective services, appropriate discharge disposition, and prioritization of the allocation of hospital resources

  • Monitor operating statistics for the Department adjusting staffing, supplies or capital expenditures

  • Recommend the annual capital, salary and non-salary budget for the Department of Care Management

Minimum Qualifications

  • Bachelors in Nursing - Master's Degree Preferred

  • Current Licensure as a registered nurse (RN) in Massachusetts

  • Five or more years of progressive experience in a hospital / health system setting in a leadership role in case management or a related role

  • Basic knowledge and application of statistical analysis, DRG's/ICD9/10, Medical Necessity criteria, JCAHO and CMS requirements for discharge planning and utilization management
