Monroe 2-Orleans BOCES SE- Transition Specialist (Part and Full-Time positions, 10 months/yr.) in Spencerport, New York

Coordinates, plans, prepares and monitors the development of a Transitional Planning Document for students with special needs who are completing their Special Education Program. Provides training and technical assistance to high school staff, parents and students on issues related to transitional planning.

TYPICAL DUTIES:

  • Identify students in need of transition services and transition team members for each student.

  • Educate students, parents, school district staff and community service providers about transition and train each group to develop skills to assume their role in the transition process.

  • Provide vocational assessment and counseling support services to students and their families.

  • Schedule, convene and facilitate transition IEP meetings, ensuring maximum involvement of the student, parent and community providers.

  • Develop transition IEP goals, objectives and activities that present student/family goals which are useful to school and agency staff and comply with state and federal mandates.

  • Collaborate with community service and resource providers to ensure appropriate and effective linkages for students with disabilities.

  • Provide professional consultation regarding students who fall under the IDEA regulations.

  • Perform other such duties as may be assigned by the Special Education Supervisor.

Job Qualifications

REQUIRED:

  1. Two years, minimum, experience working with individuals with disabilities.

  2. Knowledge and experience in transition planning for students with disabilities, job placements and job development at the secondary level.

Must meet the requirements for, or own the title of, Vocational Rehabilitation Counselor (a Civil Service position). Requirements for the title include: Bachelor’s degree in Human Services, Sociology, Psychology, Social Work or a closely related field plus two years of experience as an employment specialist or job counselor working with the disabled or in a closely related field.

OR

Hold NYS Certification as a School Social Worker or School Counselor.
