Description Lutheran Social Services of WI & Upper MI is currently seeking a Service Coordinator to serve our tenants in the Sparta Arms Apartments in Sparta and the Tom Woodman Villa in Richland Center. The Service Coordinator program is designed to promote and support self-sufficiency and independence of those individuals residing in properties, facilitating a variety of support services within the residential community. Our Service Coordinators connect residents with resources in the community, provide monthly presentations on issues in the community, and connect residents with recreational programming. This is a 36 hour and benefit eligible role , the position will be located onsite on the property! The position would be based in Sparta and require travel every other week to Richland Center. Under the direction of the Program Supervisor, the Service Coordinator HUD provides information and referrals to community connections to tenants following guidelines, as defined in the HUD Housing Handbook Section 202, who need supportive community services to maintain self-sufficiency.
