Description:

St. Joseph of the Pines is currently seeking a full-time Clinical Care Coordinator for The Health Center, our Skilled Nursing Facility location in Southern Pines, NC. Under the direct supervision of the Director of Nursing, the Clinical Coordinator will coordinate nursing services for an assigned group of residents and will apply nursing process in coordination with caregivers for a specific group of residents.

Duties include, but are not limited to:

  • Perform/manage resident assessment by evaluating physical and psychosocial status and documenting according to established procedures.

  • Ensure that residents are admitted and documentation is completed according to policy.

  • Identify resident problems, develop care plans and document actions to correct problems.

  • Develop and implement plan of care by ensuring for each resident that it is initiated, reviewed, updated and documented appropriately.

  • Prescribe nursing interventions, which are individualized and are related to specific resident outcomes.

  • Formulate, update and, implement discharge plans for assigned residents and document same.

  • Incorporate recommendations of consultants in plan of care.

  • Accept leadership for intervention in emergency situation and troubleshoot problems that may arise.

  • Administer medications according to policy and practice safety techniques.

  • Delegate appropriate care to the RN Supervisor, LPNs and CNAs and supervise/monitor their activities.

  • Evaluate resident outcomes by assessing and recording resident response to nursing interventions.

  • Anticipate and interpret the psychosocial and psychological reactions of residents to nursing and/or medical interventions and respond according to standards of care.

  • Assist in updating the resident care plans as instructed by the MDS/Care Plan Coordinator and/or Director of Nursing.

  • Collaborate with resident, family members and health team to facilitate quality care by serving as a resident advocate based on resident needs.

Education and Experience:

  • Must be a graduate of an NLN accredited School of Nursing or equivalent.

  • Must hold an ADN diploma or BSN.

  • Must be currently licensed as a Registered Nurse in the State of North Carolina

  • Must have at least 1 year of recent long term care experience.
